PCP Communication

PCP Communication History




  1.  PCP Communication Tab: The active tab will highlight to indicate what tab you are working inside. Simply Clicking any of the other tabs will navigate you to the appropriate section
  2. PCP Communication History: This table provides information on PCP communication history date, description, and status. 
  3.  Search: Use the Search to filter needed items from the history to narrow your needs
  4. Add: Click on the Add PCP Communicaton Button to add a new PCP Communication Input
  5. Action: From here you can view and print PCP communication.

PCP Communication Input 








        Communication Input: Here is where the User can insert the information needed to connect the Patient with their PCP
  1. Communication Reason: From this table, you can input a PCP Communication.  You can select multiple reasons for the communication or select 'Other' and use the free-text box to write a description. 
  2. Critical Labs: Here you can write a summary in the free text box. 
  3. Dates: In this section, you are able to select the appropriate dates for each assessment, plan or other tests.  
  4. Additional Contact Information: This section allows you to fill in the Provider name and phone number, Clinical liaison name and phone number or other contact name and number. 
  5. Communication Response: Here you can enter the PCP referral date and response to PCP referral in the free text box. 
  6. Communication Status: You can check mark Mail or Fax for the status and in this section, you are able to sign and date. The signature can be edited by clicking underneath the signature text. 
  7. Submit/ Reset: You can Submit your Fax Communication or Reset the fields 

Current Diagnosis and Active Meds




  1. Current Diagnosis: This table will provide an overview of active Diagnosis with precedence for the patient. 
  2. Active Medications: This table will provide an overview of active medications for the patient.
