Questionnaires

Overview

Keep in mind: Some elements may be slightly different depending on how your organization has set the Axiom environment up. Ex: Arrangement of Tab Sets, arrangement of categories within a page, or pages available to you based on roles and securities.

The default location of the Quick Access Panel will be the left-hand side.

1. Click "PHQ" to expand the PHQ card.
   
2. Click the "New Record" button to enter a new PHQ entry
   
3. Select which PHQ you would like to fill out: PHQ-2, PHQ-4, or PHQ-9
   
4. Respond to each question. Any relevant scoring will be calculated automatically at the bottom. (Required questions will be marked so.)
   
5. Click "Submit" to save the entry as complete.
