Clinical Form Search


1. Patient: Search for the patient’s full name. If you input only the first or last name, it will display multiple clients, we recommend inputting the whole name to narrow the search results.
2. Employee: Depending on your security, the field may be grayed out and only display your name. If you DO have the edit security, you will be able to input any employee’s name. 
3. Provider: Select the Site the service encounter was listed under. This field is not a required field.
4. Payor: You can select the Patient’s Payor. This field is not a required field.
5. Progress Note Type: Select the specific note type you are searching for; this is not a required field BUT it will help narrow the search results. 
6. Start and End Date: The system will default on the Start Date to be 5 days prior to the current date (which will be the End Date). You can modify the dates to broaden your results. 
7. Checkboxes: You can select the “Pending” and/or “Show Dynamic Forms” to include in your results. 
8. Search and Reset: Once you have completed the parameters above, you can click “Search” to review your results. Click “Reset” if you would like all the fields to be wiped. 

9. After you click “Search”, the system will open a module called “Encounter” and it will prompt you to your results based on the Criteria provided. 


10. The Encounter Module will display your results in a table. On the top right-hand corner, you refresh or download the results. The table will have the following column:
a. Patient
b. Patient ID
c. Service Site
d. Start Date
e. End Date
f. Entry Date
g. Employee
h. Service Code 
i. Service Name
j. POS (Place of Service)


11. To view the Encounters, please check off the box next to the line item (you can select multiple to view at the same time). Click “View” to review the encounter information.
