Member PCP

1.  Default Pharmacy: this is a drop-down menu that has default pharmacies to select from. 
   
2. Address: once a pharmacy is selected from the default pharmacy the address will automatically update on the table below. 
   
3. Find a Pharmacy: this is a button that will open a search option for a pharmacy not shown in the default pharmacy section. 

PCP Information

1. PCP information: this section allows you to input PCP first and last name, address, and phone number.
   
2. Comment: this is a free text box that allows the input of any comments related to PCP input. 
   
3. Submit/Reset: this allows you to submit or reset the PCP entry.
   
4. Print: this allows you to print the PCP information.
