Patient Note History

Overview

To utilize the Patient Note History Card, navigate to any progress note page (Psych Progress Note, Psych Eval, etc.) and scroll to the bottom of the page. It will be the very last card on the bottom left.

Label 1: To add information in the Patient’s Note History card, please click the three little dots in the upper right-hand corner of the card. Once you do a pop-up will appear and will allow you to enter information.

Label 2: Insert any past psychiatric and medical history in the text box below. 

Label 3: Complete the Substance Abuse History Input Assessment. 

Label 4: Complete the Social History Input Assessment. 

Label 5: Input any family history and past surgeries/hospitalizations. 

Label 6: Click Submit to update the card.
